HIPAA Privacy Statement

This Notice of Privacy Practices ("Notice") describes how Renovia Health, Inc. ("Renovia Health," "we," "us," or "our") may use and disclose your Protected Health Information ("PHI") to carry out treatment, payment, or healthcare operations and for other purposes that are permitted or required by law. This Notice also describes your rights regarding your PHI. We are required by law to maintain the privacy of your PHI, provide you with this Notice of our legal duties and privacy practices, and to abide by the terms of this Notice.

We may use and disclose your PHI for the following purposes:

• Treatment. To provide, coordinate, or manage your healthcare and related services, including communication with healthcare providers about your treatment.
• Payment. To obtain payment for healthcare services provided to you, including billing, collection activities, and sharing PHI with insurance companies or other parties as permitted by law.
• Healthcare operations. For quality assessment and improvement, case management, accreditation, licensing, credentialing, medical reviews, audits, and legal services.
• As required by law. When required to do so by applicable federal, state, or local law.
• Public health and safety. To prevent or control disease, injury, or disability; report child abuse or neglect; report reactions to medications or problems with products; and notify persons who may have been exposed to a communicable disease.
• Health oversight activities. To health oversight agencies for activities authorized by law, such as audits, investigations, inspections, and licensure.
• Judicial and administrative proceedings. In response to a court or administrative order, subpoena, discovery request, or other lawful process.
• Law enforcement. For law enforcement purposes as permitted or required by law.
• Research. For research purposes when authorized or permitted by applicable law and subject to appropriate privacy protections.
• Organ and tissue donation. If you are an organ donor, we may disclose your PHI to organizations that handle organ procurement, transplantation, or donation.
• Workers' compensation. For workers' compensation or similar programs that provide benefits for work-related injuries or illnesses.
• Military and veterans. If you are a member of the armed forces, we may disclose your PHI as required by military authorities or applicable law.
• Inmates. If you are an inmate, we may disclose your PHI to the correctional institution or law enforcement official having custody of you, as permitted by law.

You have the following rights with respect to your PHI:

• Right to inspect and copy. You have the right to inspect and obtain a copy of your PHI that we maintain, subject to certain exceptions. We may charge a reasonable, cost-based fee as permitted by law.
• Right to amend. You may request an amendment to your PHI if you believe it is incorrect or incomplete.
• Right to an accounting of disclosures. You may request an accounting of certain disclosures of your PHI, subject to applicable legal limitations and exceptions.
• Right to request restrictions. You may request a restriction on certain uses or disclosures of your PHI. We are not required to agree to every requested restriction, except where otherwise required by law.
• Right to request confidential communications. You may request that we communicate with you about your PHI in a certain way or at a certain location.
• Right to a paper copy of this notice. You have the right to receive a paper copy of this Notice, even if you have agreed to receive it electronically.
• Right to be notified of a breach. You have the right to be notified following a breach of your unsecured PHI, as required by applicable law.

Renovia Health, Inc. is committed to protecting the privacy and security of your PHI. We take appropriate administrative, technical, and physical safeguards designed to protect PHI and electronic PHI in accordance with applicable law, including the HIPAA Privacy and Security Rules.

We reserve the right to change the terms of this Notice and to make the revised Notice effective for PHI we already have about you, as well as any PHI we receive in the future. We will make the current version of this Notice available on our website, joinrenovia.com, and will include the effective date.

If you believe your privacy rights have been violated, you may file a complaint with Renovia Health's Privacy Officer or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

In addition to the privacy practices described in this Notice, Renovia Health, Inc. complies with applicable federal and state laws relating to the privacy and protection of health information. Certain states may provide additional protections or rights relating to medical or health information. Depending on your state of residence and the services you receive, additional privacy protections may apply.

California residents

California residents may have additional rights and protections under applicable California law, including the Confidentiality of Medical Information Act (CMIA), where applicable.

Other state-specific protections

Residents of certain states, including New York, Texas, Florida, Illinois, Massachusetts, and other jurisdictions, may have additional rights or protections regarding specific categories of health information. Renovia Health, Inc. will comply with applicable state and federal privacy requirements.
